Update installation with new formsPlayer branding

The branding in the installation should be updated to match that used in the new website skin. Specifically, the following should change:

• The icon that is used for Add/Remove Programs;
• the shortcut icon that is placed into the user's Programs menu;
• the application icon;
• the start page and;
• the about dialog.

Note: The application icon is currently hard-coded, so this will require enabling it to be set dynamically from the application-level meta parameters.
